A recent study conducted by UPMC Health Plan and RxAnte, a UPMC Enterprises portfolio company, has revealed the significant benefits of a high-touch Pharmacy Care Management (PCM) service for high-risk Medicare Advantage members. The study found that this targeted program, which includes pharmacist care management, medication reconciliation, and home-delivered medication in easy-to-use packaging, not only improves medication adherence and member satisfaction, but also lowers net costs.
As Medicare Advantage health plans see an increase in enrollment among medically complex and vulnerable populations, managing the cost of care is becoming more challenging. Traditional pharmacies are not meeting the clinical needs of this complex and vulnerable senior population. However, the PCM program, provided by Mosaic Pharmacy Service, a subsidiary of RxAnte, offers a comprehensive and consistent pharmacy care solution.
